Our Services

Person-Centered Planning

PCP is about the individual's future and reaching their goals. The PCP process leads to a written plan that is consistent with the person's needs and desired outcomes. The individual choices decided through PCP about what the person wants and needs and what services and supports will help them reach their goals is used to inform their Individual Program Plan (IPP).

Ongoing Independent Facilitation

Pre-enrollment Transition Support

• Develop Spending Plan based on approved budget

• Help finding services / resources

• Onboarding with Financial Management Service (FMS)

• Advocating on behalf of client in development of the IPP

Self-Determination Program participants receive one-time funding from Regional Center for PCP development and transitioning into SDP program. Participants do not pay for these services directly.

• Facilitation with FMS agency

• Spending Plan updates

• Other tasks that client need support

My journey began 20 years ago changing my career from biotechnology researcher to full time advocate for my son. I have first hand experienced the challenges families face in getting appropriate services for their loved ones. SDP has been life changing for our family.

I have received my certification as Independent Facilitator and have attended many workshops and seminars for Person-Centered Planning and Independent Facilitation. I am here to help families and individuals to successfully enroll in Self-Determination program.
